Office hours are by appointment. Our office is open on Monday through Thursday from 8:00 AM until 4:30 PM and on Friday from 8:00 AM until 4:00 PM. Please contact us for available appointment times for each provider.

Please observe the following appointment guidelines:

• 24 hours notice of cancellation is required (48 hours for cosmetic, elective, and surgical appointments).
  
  
• New patients must arrive 15 minutes prior to their appointment to allow time for chart preparation. (Established patients with changes to personal, medical, or insurance information must also arrive 15 minutes early to update their records.)
  
  
• Minor children (under 18 years old) must be accompanied by a parent/guardian at the first visit or whenever you have questions regarding treatment.
  
  
• Charge card information is required to hold appointment time for cosmetic, elective, and surgical appointments. If the time is missed without notice of cancellation, a charge will be accessed.
  
  
• Whenever you miss multiple appointments without notice of cancellation, a charge card will be necessary to hold any future appointment times. If time is then missed without notice of cancellation, a charge will be accessed.
